Quick answer: A trouser leg twists when the crease and the side seam swing round the leg instead of hanging straight, and the cause is one of four things: the cloth grain, the way the leg was cut, the way the seams were eased together, or the client's own leg line. Photograph both legs from front and side with the client standing naturally, note whether one or both legs twist, and send the photos with the cloth reference so the cause can be read before anything is recut. A client tries on the trousers and the front crease, which should fall to the middle of the shoe, drifts towards the outside on one leg. The side seam wraps forward. After pressing it comes back. Twisting is one of the more frustrating trouser faults because it can come from the cloth, the factory or the person wearing them, and the fix depends on which. A clear report is worth more than a guess.
Why does my trouser leg twist?
Because the leg is not hanging on the straight grain of the cloth, or because the body under it is not straight. Cloth has a lengthwise grain; a trouser leg is cut so the crease runs along it. If the cloth was woven or finished slightly skewed, if the pattern was laid a little off grain, or if one seam was stretched while being sewn to the other, the leg turns as it hangs. If the cloth and sewing are right, a leg that turns in or out at the knee or foot will still pull the crease with it.
What causes it
Skewed cloth: both legs twist the same way and stripes run on a slant — look at the weave or stripe at the hem.
Leg cut off grain: one leg or both, from the first wear — the twist stays when the trousers hang on a hanger.
Uneven seam easing: the side seam wraps forward and the inseam pulls — the seams pucker or one seam looks tight.
Crease pressed off grain: the crease runs off-centre at the hem — re-pressing on the grain improves it.
Client's leg line: one leg, with the foot turning out or the knee turning in — the twist appears on the body, less on a hanger.
The hanger test
Hang the trousers by the hem, clipped at the crease, and let them fall. If the legs hang straight on the hanger but twist on the client, the body is the main cause and the pattern needs a leg line note; see knock knees and bow legs. If they twist on the hanger too, the cloth, the cutting or the sewing is the cause, and it is the factory's to correct. Thigh size can also pull a crease; see big thighs.

Which leg: left, right or both.
Which way: crease swings to the outside or the inside.
Where it starts: from the seat, the knee or near the hem.
Hanger result: twists on the hanger, or straight on the hanger.
Photos: front and side of both legs, client standing naturally, feet as he stands, plus the hanger photo.
Cloth: reference, and whether it is a stripe or a plain.
What the factory checks
Cloth grain before cutting, the grain line on each leg piece, matching of stripes and checks along the leg (see stripe and check matching), even easing of side seam and inseam, and the crease pressed along the grain. On a bulk run, the same leg measurements and twist are checked against the approved sample; see measurement tolerance and stitch defect inspection. A crease that is chemically set will hold any twist with it; see pressed or set creases.
