Quick answer: When one shoulder sits lower, the jacket should be cut lower on that side too, so both sides hang the same on the body. Spot it in a mirror and a back photo, write which side and whether it is slight or clear, and check it on the sample before the pattern is sealed. Shi Yuan (Suit Factory) takes MTM from 1 and bulk from 50 per style for overseas tailor shops, MTM boutiques and private-label brands, not retail — send the back photo and which side is lower on WhatsApp.
On the sample the left side looks right. On the right, the shoulder droops past the bone, a fold runs from the neck to the armhole, and the hem hangs lower. The jacket was cut the same on both sides for a body that is not. Most people carry one shoulder lower than the other, and a symmetrical pattern shows it.
Made to measure fixes it, as long as the order says which side and how much.
How should a suit shoulder fit when one shoulder is lower?
On both sides the jacket shoulder should end at the shoulder bone, sit flat without a dent or a ridge, and let the sleeve hang straight. With a low shoulder, that means the pattern on the low side is dropped to meet the body. Padding the low side up to match the other is a different choice: it hides the difference but can look stiff. Write which you want.
Signs on the low side
Shoulder line: seam droops past the bone or dents near the sleeve — the slope is cut too square for that side.
Neck to armhole: a diagonal fold running down and out — extra cloth with nowhere to sit.
Collar: a gap or roll on one side of the neck — the collar is sitting on a shoulder that is not there.
Hem: that side hangs a little longer — the whole side has dropped.
Sleeve: the cuff looks longer on that side — often the shoulder, not the arm.
How do you measure it at the counter?
Ask the client to stand as he normally stands, arms relaxed, weight on both feet. Do not let him straighten up for the tape.
Take a back photo at shoulder height, square to the client, with a plain wall behind.
Look at the shoulder points against a horizontal line on the wall or a door frame. Note which side is lower.
Measure both sleeves. If one sleeve measures longer, write both; do not average them.
Write the result in words: "right shoulder lower, slight" or "right shoulder lower, clear". The pattern maker reads the photo with the words.
The full photo set we ask for at sample stage is in the sample feedback photo set.
Drop it or pad it?
Drop the low side. The pattern follows the body. The jacket looks natural and moves well; a close look shows the shoulders are not level, as the body is not.
Pad the low side. A thicker pad lifts the low side towards the other. The shoulders look level, but a heavy pad on one side can feel and look built-up, especially in a soft-shoulder house style.
Both, a little. A small drop and a little extra pad is a common middle way. Write it as "drop right slightly, light extra pad right".
How shoulder style changes the choice is covered in soft vs structured shoulder; a dent near the sleeve that comes from the sleeve head rather than slope is in shoulder divots.
The order note
"Client ref A-12. Right shoulder lower, slight (back photo attached). Drop right shoulder to body; no extra pad. Right sleeve measured longer, both sleeves as measured. Check collar on right side and hem level at sample."
If the collar stands away on the low side even after the shoulder is corrected, the collar itself may need a note; see suit jacket collar gap. A client who also carries a fuller stomach needs the front balance notes from suits for men with a belly.
Which track handles a low shoulder?
MTM — from 1 piece: the low-shoulder note goes on the client's record and every later suit starts from the approved sample.
Bulk — from 50 per style: a size run is cut symmetrical; a brand handles low shoulders through alteration at the shop or by sending that client through the MTM track.
How the steps run from measurements to sample to production is on our process page. The sample takes 7–14 days; production takes 14–21 days after measurements, cloth and construction are locked. Production days are not delivery days. The MTM track is on our made to measure suit factory page.

Can a low shoulder be fixed after delivery? Sometimes a little at the shoulder seam; cutting it right is far easier.
Should the trousers change too? Sometimes the hip is lower on the same side. If one trouser leg looks longer, measure both legs.
