Quick answer: Surgeon cuffs are working sleeve buttons with open buttonholes so the cuff can actually unbutton; dummy sleeve buttons are sewn on for show with holes that do not open — the shop must choose which, and lock button count and spacing, before the sample. Write working or dummy, the button count, and whether kissing or spaced, then confirm on the sample that holes open or stay closed as briefed. Shi Yuan (Suit Factory) takes MTM from 1 and bulk from 50 per style for overseas tailor shops, MTM boutiques and private-label brands, not retail — send the working-or-dummy choice and button count on WhatsApp.
"Surgeon cuffs" sounds like a construction upgrade. On an order form it is a yes-or-no: do the sleeve buttons work, or are they dummy? Private-label brands that sell a "handmade cue" often want working holes. Hire fleets and some bulk programmes prefer dummy buttons because working holes add handling and client misuse risk. The companion post that locks how working holes are proven on a sample is working cuff buttonholes sample lock. This page is the when-to-choose brief.
What are surgeon cuffs on a suit?
Surgeon cuffs means the cuff buttons can unbutton through real buttonholes — historically so a surgeon could roll back a sleeve. On a modern suit it is a finishing choice and a talking point for MTM clients. Dummy buttons sit on the cuff with no functional hole, or with holes stitched shut. Neither choice replaces button count and spacing; those are in sleeve button count and spacing. Button material and buttonhole specs are in button and buttonhole specs.
When to choose working vs dummy
MTM client who values working details: prefer working (surgeon) cuffs — matches the story he is buying.
Private-label brand with a make cue in the lookbook: prefer working, sealed on the sample — consistency across the size run.
Hire fleet or hard-wear programme: prefer dummy buttons — fewer snags and less client fiddling.
Soft unstructured jacket with minimal make cues: prefer dummy or fewer buttons — keeps the cuff quiet.
Unsure: ask on WhatsApp before cutting — do not assume from a competitor photo.
Spec lines
Function: "working surgeon cuffs" or "dummy sleeve buttons".
Count: button count as a word on the brief.
Spacing: kissing or spaced, per the count post.
Thread: button thread and shank note if the brand has one; thread shank sewing is a separate construction brief.
Sample proof: "working holes must open on the sample" or "dummy — holes not open".
A worked note
"Brand ref A-12. Sleeve: working surgeon cuffs, four buttons kissing as house style. Sample must open. Spare sleeve buttons in the spare set."
At the sample
Try to unbutton the cuff. If briefed working, every hole must open cleanly.
If briefed dummy, confirm buttons are secure and holes are not open.
Check count and spacing against the sealed sketch.
Check both sleeves match.
Note any tight hole that will fray when the client forces it.
Client communication tip
Working cuffs are a talking point on the shop floor. Tell the client whether holes ship open or still basted, and whether he should unbutton them day to day. Many men never open surgeon cuffs; that is fine, but the holes must still match the brief on the sample. Dummy buttons need the same honesty: do not call them surgeon cuffs on the hangtag.
How sleeve finishing sits in the make sequence is on the process page; button and trim locks before bulk are in the button and trim lock.
Which track needs a cuff-function lock?
MTM — from 1 piece: one client's jacket with working or dummy cuffs written on his order.
Bulk — from 50 per style: a brand's style run with working or dummy sealed on the sample before bulk from 50.
The sample takes 7–14 days; production takes 14–21 days after measurements, cloth and construction are locked. Production days are not delivery days. How cutting, sewing and finishing follow each other is on our process page, and how you approve the sample on our sample policy page.
